That all looks in pretty good condition , tbh. 
Have you tried adjusting the ends.(see post above)
It just sounds as if your cables have stretched slightly, and screwing the ends in a couple of turns may solve all your problems.

Ah! Didn't spot that. 
I think it was Dieter(I'll check) who took the joints off and remade them replacing the damaged rubber with silicone rubber.

...but to answer your question, you just need the Mike Satur bellcrank. Your rubber boots look in good condition.(Unless I can't see the damage!
)
